CALIFORNIA HEALTHY KIDS SURVEY The California Department of Education (CDE) funded the California Healthy Kids survey (CHKS) to assist schools in preventing youth health-risk behaviors and in promoting positive youth development, resilience, and well being. ABC Unified School District surveys all 5th, 7th, 9th, and 11th grade students every two years as required by the Safe and Drug Free Schools and Community Entitlement along with the Tobacco Use Prevention Education Entitlement. Active consent is required for 5th grade students. Parents must turn in a completed permission slip for their child to participate in the survey |

Passive consent is required for 7th, 9th and 11th grade students. Parents must sign a withdrawal form to remove their student from participating in the survey. Both parent permission slips and withdrawal forms are sent home before the survey is taken.
Parents may view the questionnaires, by appointment, at the Drug, Alcohol, and Tobacco Education (D.A.T.E.) district office by calling (562) 926-5566 Ext. 21117 or online at www.wested.org/pub/docs/chks_summary.html. (Click Elementary, or Module A for Middle school or High school.)
